COVID-19 Visitor Declaration

In light of the current COVID-19 pandemic we ask you to please comply with the following arrival/induction requirements.

In attending Mercedes College, you declare that you:

Have not been in any location identified by SA Health as an active potential exposure site for COVID-19 (Current COVID-19 SA Health Alerts); Have not returned from overseas within the past 14 days; Have not been in close contact with a confirmed COVID case within the past 14 days (defined as someone who has more than 15 minutes of face to face contact or has spent more than two hours in a room with a confirmed COVID case without appropriate PPE);

Are currently well and not showing any flu like symptoms such as:

• fever
• sore throat
• shortness of breath
• coughing
• fatigue.
